At Bampton Medical Practice, we are working to ensure that all patients with a military background are correctly identified in their medical records. Once confirmed, an electronic note will be added to your record stating your Military Veteran status. This is important, as it will be included on any referrals your GP makes to hospitals and other services.
If you have left the Armed Forces and are returning to civilian life by registering with an NHS practice, please provide full details of your service. This enables us to request a copy of your military medical records so your care is joined up and nothing is missed.
