Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter -reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved. Patient Registration
To request registration patients should live within the Practice catchment area (download map here) and are required to complete the new online registration process as detailed below.
Every patient (including children) who is registered at Bampton Medical Practice is allocated a named GP
We can now offer online Registration for patients
We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

IMPORTANT:
When you register online you will also need to download and fill out a New Patient Registration questionnaire. We need this because it can take a considerable time for us to receive your medical records. This Questionnaire provides us with all the important information the practice needs in order to complete your registration.
